Therapy Agreement


After an initial assessment you will be asked to commit to the minimum of six sessions when the work will be reviewed. The review will clarify progress and identify further work. A further contract will be agreed at the review identifying your particular needs.


Your therapy time is reserved for you; therefore unless there is a real emergency I expect 7 days notice and expect to be paid in full if you cancel in less than 7 days. I will always try to reschedule another session if you are unable to keep your appointment. However if this is not possible I will charge for the session. If you are late the session will finish at the agreed time. I will require one weeks notice if you cancel the sessions prior to the arranged time.


Ending therapy is discussed at regular reviews. If you are not happy about any aspects of the therapy I would encourage you to talk it through with me before finishing the therapy. It is important that therapy feels finished and complete, which usually takes two sessions at the end of our work together.


I am bound by C.O.S.R.T professional code of ethics regarding good practise, which require all sessions to be strictly confidential. No information will be shared without your permission except under exceptional circumstances or unless required by law such as as the Terrorism Act and Drug Trafficking Act [where I reserve the right to contact who I think would be appropriate if I believe you are in danger of harming your self or harming others]

From time to time I will discuss the work with my supervisor for the purpose of consultation. This will always be done on an anonymous basis.

Any notes taken are kept in a locked filing cabinet in the office in my house and destroyed after 7 years. The notes will not identify you or anyone that you mention by name, They are to enable me to keep track of the number of sessions, the content of sessions, in no way are they official psychological assessments or reports. They are hand written and there is nothing stored on my computer or on memory stick. Personal details held are first names and telephone only and are stored separately in my business mobile and deleted as soon as sessions have finished. The brief hand written notes are not shared with anyone.


Should you telephone or otherwise contact me to inform me of suicidal or homicidal intentions, I reserve the right to contact the emergency services. Equally if during a session I believe you are expressing intention to harm yourself or another person physically or sexually or are unable to take responsibility for your actions due to mental illness, or disclosing a criminal offence i reserve the right to seek further professional guidance and to contact your GP. In all instances I will  discuss this with you first, before contacting any third party.

If you are involved with Social Services, probation or mental health professionals, I may want your permission to inform them that we are working together.


I work according to the code of ethics laid down by the UKCP and COSRT complaints procedure. I am registered on data control ICO Reg No ZA033201


Should I meet you outside of your session please be aware that I will not greet you. This is to protect your privacy and maintain confidentiality. This is especially important if you are with other people. However if you decided to acknowledge me then I will respond in an appropriate manner. 

I do not respond to ant social media, face book etc or respond to emails or tel calls outside of the session.
